Description

Qualifications:
1. A bachelor’s degree from an accredited four-year college or university with a major in social work, psychology, sociology, or closely related field;

     OR
    A bachelor’s degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency.


2. Valid Minnesota Driver's License

Job Summary:
Under direction, performs responsible work on cases involving complex client needs; develops, implements, and monitors case plans and performs related work as assigned.

Duties:
This position provides case management duties with a primary focus of working with those who have developmental disabilities and receive county, state, or federally funded services.  The case manager creates, implements, and monitors detailed service plans to manage goals, services, and resources specific to each client.  Provides education to individuals, family members, and community partners regarding programs and services available; refers clients to a variety of support services.  Maintains detailed documentation of participation in case records, ongoing case narrative, adheres to program deadlines. 


Hours:
Full-time, Exempt; 40 hours per week on average

 

Full Salary Range and Benefits:
$69,535 - $90,740 annually; starting wage DOE, the position is eligible for a wide range of competitive benefits, including state pension plan, vacation time, sick time, personal and holiday leave time. In addition, the position is eligible for the County's health plan, consisting of three health plans and carrier options; Dental, Vision, and Life Insurance; Health Savings and Flexible Spending accounts; Long-Term and Short-Term Disability; Supplemental Accidental, Hospital, and Critical Illness plans; and Public Service Loan Forgiveness (PSLF) program. The County contributes monthly health benefits to aid in the rising of insurance costs with the contribution rate evaluated annually. Additionally, employees have the option to take the $ 734.15 contribution as cash in lieu of benefits. A telecommuting policy is in place and may be available after the initial probationary period.
